A.7. Problem/Solution Worksheet

There are three worksheets in this appendix:
a. Problem/Solution Worksheet I, Part A and B
b. Problem/Solution Worksheet II, Part A and B
c. Final Session Review

These worksheets are to be used in conjunction with the counseling sessions. After the initial review of personal data and after every subsequent counseling session, The counselor should use Problem/Solution Worksheet I to organize his observation and analysis (in Part A ), and a plan of action (in Part B ). Then he can follow up with Problem/Solution Worksheet II to lay out topics to be discussed during the next session. The counselor can either use Worksheet II, Part A, as a handout to the counselee, or write up a more formalized page for the session as he/she wishes. Worksheet II also includes a homework section in Part B . Counselee should be expected to complete the homework assignment to demonstrate his/her commitment to work on the problem area. Final Session Review is used in the last session to lay out a future direction for the counselee.

Problem/Solution Worksheet I

Review counselee's personal data sheet or results from last week's homework. Make note of judging others, blame shifting, self-will, weak or no commitment, lack of sensitivity to sin, defensiveness, bitterness, sin patterns ( Gal. 5:16-24 ; Col. 3:5-15 ).
